There are several forms that make up the new client packet that you will need to complete. We have tried to make this process as easy as possible by having our forms available to complete online below.
If it is difficult for you to complete the paperwork online, please call us. We can complete the forms together over the phone.

NOTE: all of our forms ask for your email as the first question. This is done so that a copy of your responses and completed forms can be sent immediately to you for your records.
The New Client Packet (for Non-Massachusetts Residents)
Basic information—name, address, phone, email, etc.
Life, status, health, & mental health assessment questions
Do not complete this form until after the initial assessments have been completed and you and your counselor have agreed to continue working together. Typically this happens in the second or third session.
- Policy Statement to sign (usually done by the 2nd session)
- Read & download our Policy Statement, Privacy Notice, and HIPAA Statement (non-MA residents)
- Read & download the REAT Code of Ethics of the International Expressive Arts Therapy Association (IEATA)
- Read & download our Client Bill of Rights
